Photo Request Form

It is agreed that photographic reproductions will be purchased under the following conditions (see attached statement for further details):
1. Payment in full is required prior to obtaining reproductions.
2. Reproduction arrangements will be made or approved by Museum personnel.
3. Copy negatives, digital scans, and/or slides are the property of the Museum.
4. Photographs are to be used one time and only for the purpose stated above. Additional reproduction of photograph(s) must be approved in writing by the Director or Curator of
History.
5. Photographs will be identified with titles/captions provided by the Museum and must be credited in all publications and exhibits as follows: From the Collection of the Loveland
Museum.
6. The Museum shall receive a copy of resulting publications, etc. without charge.
7. Please allow up to 2 weeks for order to be processed and filled.
8. If images are requested for research purposes only, low resolution copies or scans will be provided for up to 6 images. If more than 6 images are being requested, additional images may be ordered using this form for $1.00 per image.
